Payment
All Co-payments are due at the time services are rendered. If you have Medicare but do not have secondary coverage you will be required to pay for 20% of your visit. We accept cash, check, and money orders. We do not take credit cards. We will charge a $25.00 fee if the check is returned as insufficient.
Insurance
We emphasize that as a health care provider, our relationship is with you, not your insurance company. To ensure payment, you should contact your insurance company for pre-authorization to receive services in our office. It is your responsibility to understand your insurance policy for your health care benefits. While the filing of your insurance claims is a courtesy that we extend to our patients, all charges are your responsibility. If your insurance changes, it is your responsibility to notify our office. If not, you will be responsible for the bill.
Medical Records
Any patient, company, or doctor’s office that needs medical records must have a signed release by the patient. Exception: Doctors that referred the patient to us, or doctors that we have referred patients to.
Process of Releasing Medical Records
At the time we have a signed release by the patient, medical records will be processed in our office on a first in first out basis. Contact our office for current charge per page and service charge.
Disability Forms
Please allow all disability forms 7-10 days for completion. Forms need to be brought to our office, a Form Request needs to be filled out, signed and dated. We will complete the form and contact you for pick-up. Certain forms are subject to charges and all charges must be paid prior to the form being returned.
